  • Responsible: Individuals who perform the work and ensure the task is completed.
  • Accountable: The person with ultimate ownership of the task. Only one person should be accountable to avoid confusion.
  • Consulted: People whose expertise or input is required. Communication is two-way.
  • Informed: Stakeholders who need updates but are not directly involved. Communication is one-way.

Applications/Examples

The RACI model is applied in diverse industries to support project governance. In IT system rollouts, for instance, developers may be Responsible, the project manager Accountable, subject-matter experts Consulted, and executives Informed. In healthcare, RACI matrices clarify who performs clinical tasks, who signs off, and who requires updates.
