Introduction: Ishikawa Diagram

The Ishikawa Diagram, also known as the fishbone diagram or cause-and-effect diagram, is a visual tool used to systematically identify and analyse the root causes of a specific problem. Widely applied in Lean, Six Sigma, and quality management, it helps teams move beyond symptoms to uncover underlying issues.

Background

Developed by Kaoru Ishikawa in the 1960s, the diagram was originally designed to improve quality control in manufacturing. Its fishbone shape provides a clear structure for organising potential causes of a problem, making it easier to facilitate discussions and drive problem-solving.

Key Elements/Features

  • Head (Effect): The defined problem or outcome to be analysed.
  • Spine (Main Line): Represents the path from the problem back to possible causes.
  • Branches (Categories): Commonly include People, Methods, Machines, Materials, Environment, and Measurements. These can be adapted to suit the context.
  • Sub-Causes: Smaller contributing factors branching off from each main category, providing a deeper level of detail.

  • Manufacturing: Identifying causes of defects in a production line.
  • Healthcare: Exploring reasons for delays in patient care.
