The 5 Whys technique was developed at Toyota by Sakichi Toyoda and became a key part of the Toyota Production System. It was designed to be practical and easy to use, helping employees of all levels dig deeper into problems without needing advanced statistical tools.

  4. Why? – The lubrication pump was not working.
  5. Why? – The pump filter was clogged.
    By asking “Why?” five times, the team discovers the real cause is poor maintenance, not just a blown fuse.

The 5 Whys can be applied in factories, offices, healthcare, or services – anywhere problems need to be solved quickly and effectively.
